Oak Meadows is owned and operated by Family Resources Development, Inc., a not-for-profit organization affiliated with the Minnesota Conference of the United Church of Christ (UCC) and a member of the Council of Health and Human Services Ministries of the UCC.  We have a successful partnership with Presbyterian Homes & Services. On-site home care is provided twenty-four hours, seven days a week by Presbyterian Homes & Services.  Making a lifestyle change can be a challenging experience.  Oak Meadows and Presbyterian Home Care staff are here to provide the support and guidance you and your loved ones may need during this transition. Please do not hesitate to call us with any questions you may have.  We look forward to hearing from you.
